      Well, I have to agree with those who voted for

      Heroes - because Janet died; and
      Threads - because Jacob died

      Coming in after those is:
      Divide & Conquer - because Martouf died
      and the episode where Lt. Elliott died, along with Lantash

      Very sad - all of them.

        Message in a Bottle - Season 2 (About Jack O'Neill's son)
        Forever in a Day - Season 3 (Jack is stuck on a planet and Carter misses him).
        Ascension - Season 5 (When Orlin leaves Sam)
        Meridian - Season 5 (When Daniel dies/ascends)
        Revelations - Season 5 (Sam misses Daniel)
        Grace - Season 7 (Sam is traped alone on the ship)
        Heroes - Season 7 (Dr. Fraiser dieing)
        New Order - Season 8 (When Fifth tortures Sam)
        Threads - Season 8 (Sam's dad dies)
        The Powers That Be - Season 9 (Vala becomes more caring and the Ori victory)
        Crusade - Season 8 (Vala's story)
        Camelot - Season 8 (SG1 losing the battle)
        Flesh & Blood - Season 9 (Vala and her child; SG1 being defeated)
        Memento Mori - Season 9 (Vala loosing her memory)

          These are the ones that made me tear up
          Meridian - Daniel Dies
          Forever in a Day - Sha're dies
          Threads - Jacob dies
          Unending - Last SG-1 ep, overall it was just sad
          Memento Mori - The bit toward the end with Vala and Daniel
          2010 - When they are battling to get the note through the gate
          Lifeboat - Where the boy talks through Daniel to his father

            In the Line of Duty because Jolinar of Malkshur dies
            Chain Reaction: Hammond "retires"
            2010: they all die to get the message through the gate
            Between two Fires: all the tollan die
            2001: Sams alternate future husband dies
            Last Stand: lantash/ elliot die
            Meridian: Daniel dies and I think the saddest episode of them all next the heroes
            Frozen: that ancient lady sacrifices herself to heal everybody
            Full Circle: We find out about Anubis and abydos is destroyed
            HEROES"S: Janet dies but you don't know until that video
            Threads: Selmac/ Jacob dies
            Unending: The last episode and the teams mental break downs.

                  Not trying to be nitpicky here, but:

                  Cold Lazarus in Season 1 is about Jack O'Neill's son. Message in a Bottle is about an alien device that shoots a bolt through Jack's shoulder pinning him to the wall of the gateroom.

                  One Hundred Days in Season 3 is about "Jack is stuck on a planet and Carter misses him". Forever in a Day is about Daniel finding Sha're and Teal'c having to kill her.

                  Unless, of course, I missed something somewhere and didn't see the same episodes.

                                Didn't see a thread for Atlantis so I thought I'd add some:

                                Before I Sleep (S.1) - Something about seeing how it all could have turned out and seeing Weir all alone.

                                Letters From Pegasus (S.1) - All the videos being sent home. Really liked RSF's (Ford) performance in this one. He's really the heart of the team (him and Carson).

                                Sunday (S.3) - Carson
