I think this is a great idea, and vote for option 4.

Is that the first time she calls him Jack?

I don't know if anyone has answered your question or not, but Sam calls O'Neill "Jack" in Solitudes during season one. It was toward the end of the episode when she thought they were both going to die. Jack was delusional and he called out for Sara. Sam answers by saying "I'm here, Jack".
