Do they just cover the ship, sort of like a shrink wrapped bag over it, or do they create a bubble around the ship, like a warp drive does?

I don't think it's ever been made specifically clear. Though if you watch the VFX of the cloaked Bird of Prey in Star Trek III, that appears to support the 'shrink-wrapped bag' line of thinking."A society grows great when old men plant trees, the shade of which they know they will never sit in. Good people do things for other people. That's it, the end." -- Penelope Wilton in Ricky Gervais's After Life

Come to think of it, Star Trek VI also lends credence to the 'shrink wrap' theory as well.
Notice how the glow of the torpedo only illuminates where its light hits, rather than illuminating the inside of a bubble."A society grows great when old men plant trees, the shade of which they know they will never sit in. Good people do things for other people. That's it, the end." -- Penelope Wilton in Ricky Gervais's After Life
