Actually, I believe it should be "You don't cut leeches off, you pour salt on them." but maybe the quote is wrong or the character staying it said it wrong. I've heard the term used before numerous times though never with "out" in place of "off" as a leech is on the skin, not in the skin and therefore, would not need to be cut out of anything.
