For Sam's mom, I've seen a lot of people name her "Catherine". As for Hammond's wife, haven't seen her too much in the fics I've read. Has there ever been given a name for Sara's dad?

Nope, their names were never mentioned. However, in one of the novels Hammond's wife is/was called Margaret but personally I like Mary for her. The name of Sam's mother was never shared with us either, although some people say it sounds like Jacob said something along the lines of "Thea" (short for Dorothea perhaps) when SG-1 finds him on Netu - to me and many others it just sounds like Sam which makes sense since she was there and he was probably slurring a bit. In the RPG series she's called Debra Nielsen but in another novel her name (in an alternate reality) was apparently Eleanor.

Just an FYI: Many online transcripts of "Cold Lazarus" give Sara's father's name as "Mike".

1. Regarding the Goa'uld ribbon device (kara kesh), would you say this is something that only a System Lord and members of his immediate family would have? Or maybe those individuals plus their high-ranking Goa'uld lackeys -- say, senior military commanders and such? (Personally, I'm thinking at least Goa'uld at that level would have them.) Or would it be more common than that?

2. Same question, only this time regarding the Goa'uld healing device.

I think it would only be (fallen) System Lords and perhaps their most important underlords, depending on the size of their domain. Osiris had one (stashed away), so did Hathor and wasn't Bynarr allowed one (plus a staff weapon) on Netu? I think the same goes for healing devices, although they may be more readily available since Goa'uld can heal pretty well because of the snake and/or use of sarcophagus which makes the healing device almost redundant for them (especially if they're unable to use it on themselves, which has never been clarified one way or the other).
