                  ~~ It's about a girl diagnosed with cancer. she doesn't think her boyfriend will stick with her, after all, who wants a bald girlfriend? But the prom rolls around, and not only does he show, but he's shaved off his masses of hair to support her. They all break out into tears, Sarah-beth and her "very first TRUE love", as the song says, go to the prom and have an evening that Sarah-Beth only dreamed of having. The song is a what if about one of the band's daughters. one of the people in the band lost his daughter to cancer at a very young age, so the song was a "what if she had gotten it later?" story really
