I think the only members we've ever permanently banned are those who posted so far over the line that they made it clear they were only here to break the rules -- like creating an account to post porn links in their first post, then never coming back. When it comes to disciplining active members, we'll always opt to put them on moderation rather than banning them entirely.
As many members in the GateWorld Features folder can attest, it's hard to come up with a well-worded poll with good, sufficient options. I'd rather restrict poll-creation to the mods to keep our polls high-quality, rather than seeing polls like this every other day:

it should read 73 after i post this but im not sure if it will cos at least twice now its stayed on 72. and im pretty sure it hasnt worked for longer than that. if you have the time could you please unstick it? 
that way your resolution is there as well
