Yep. Death Star II was most definitely, unmistakably, certifiably, completely destroyed.
Nah. When the shot ends it's still a big fireball, so you really can't tell. (Also: each new version of the film supersedes the previous release).

Maybe so, but the Death Star's destruction is one thing that carries over in each release, with the addition of the shockwave in the 1997 and 2004 versions. Although, I've yet to see the Blu-ray release (and I don't intend to).
