      AT least sony didn't put spyware on the DVDs, otherwise our TVs would constantly change channels to Wheel of Fortune every time we turned it on....

            I went through the process but I don't see an address. It just says a P.O. Box with no number and no entity. So How and where do we mail it back?

            Anyone know?

              I just printed the non-print version of the form, and I don't get it. do we send it to Columbia Tristar or Redemption Headquarters???

                Redemption HQ I believe, although I'm confused as to if we need to send in the original dvd or just the proof of purchase. the website says
                "SEND: (1) THIS COMPLETED CERTIFICATE.
                (2) THE ENTIRE JEWEL CASE AND DVD (AND OR)
                (3) THE ORIGINAL PROOF OF PURCHASE FROM YOUR PACKAGING."

                which I read as saying the disk OR the PoP, but tvshowsondvd says to send in the DVD. I'd rather keep the original disk if possible, to watch the other episodes while waiting for the replacement to get here

                  All you have to do it mail the Proof of Purchase (on the lower right of the Disc 5 case, cut it out) and the certificate from checkmyrebate.com.

                    Umm... what the hell? Is this only available for people in the US?

                    It says 'State'... I don't see provinces anywhere...

                    What a bunch of crap. Don't they think maybe people in Canada want the disc too??

                      Of course, Canadians aren't applicable, because the online form only allows for US residents to apply for the replacement DVD. Sigh.

                            Yes, it is a little confusing as to whether we send the entire case (is it really even "jewel" case?) and all of it's contents or just the proof of purchase from the insert in that case. Can anyone find out what it actually is?
